Even if you have done everything to communicate your own end-of-life wishes, you may find yourself in a situation where you need to take the initiative and have the discussion with family members or loved ones who have not shared their end-of-life wishes with you. In an attempt to make sure your wishes are carried out, you are encouraged to complete an advanced directive.

Advance directives are legal documents that allow you to spell out your decisions about end-of-life care ahead of time. They provide a means for communicating your wishes to family, friends, and healthcare professionals, and help to avoid confusion later on.

Tasks:

Research your state's advance directives. Review and complete a copy of the appropriate state advance directives form. The form must be notarized.

In a 300 word reflective post, address the following;

• Discuss what advanced directives are and why they are necessary.
• Discuss who you identified to be your representative and why.
• Discuss the reaction of your family members when you notified them of your wishes.
• Discuss the emotions you felt when completing this exercise.
• Create a copy of an obituary (sample). The obituary should have an advanced date of death, and should include the goals you hope to have accomplished, the loved ones you would have left behind, and a brief statement about what someone else would say about you at your death.
