To apply evidenced-based practice to a skilled nursing and rehab unit a nurse must possess the skills to find the best source of evidence to support clearly formulated clinical questions. This will allow the nurse to search for relevant answers to those questions and determine when and how to incorporate those findings into their current practice setting.

For Instance, the skilled nursing and rehab unit that I currently practice in suggests that our facility must have an open reporting policy related to medications errors. Current evidenced-based practice states that health care centers that adopt this policy will have higher success rates of prompt reports of near misses and actual medication errors. The suggested practice is to evaluate the nurses surrounding and challenges they might face in direct correlation with the error that was made. This process holds the system to a higher accountability then the individual who made the mistake. Evidence-based practice supports the conclusion that when nurses are in fear of disciplinary action they are less likely to report medication errors. In certain circumstances, the delay in reporting these errors have led to direct patient harm and even preventable deaths.
