Follow these steps to search for your CHNA. Please do not hesitate to contact me if you cannot locate this report. Email me with a best number, day, and time to contact you.
This is a large document and there is no need to download it. However, familiarize yourself with the chapters or sections. This report contains qualitative and quantitative data collected to help the healthcare facility develop a strategic plan that promotes and supports community health.

Step 2: Answer the following in a 3-5 paragraphs and reference page in APA formatting. Your sources should include, at least, the course text, the CHNA from the hospital website, and Office of Disease Prevention and Health Promotion website (Healthy People 2020).
1. Identify vulnerable populations noted in your windshield assessment and the
hospital’s CHNA. Do you agree or disagree with these populations? You might not have observed direct indications of these residents, but could your infer them from indirect observations?

2. Compare any vulnerable populations noted in your windshield assessment
and the hospital’s CHNA. Discuss briefly how Health People 2020 relates to these populations (discuss no more than at least 3 populations).

For example, you observed several adolescents gathered around a local mall leaning against buildings, smoking and playing loud music during after school hours. Based on you observations, you suspect these adolescents are likely to have their health compromised due to lack of locations for recreations and smoking hazards. These health needs can be found in HealthyPeople2020 with goals for reduction, interventions and educational information.
