Child abuse is a big problem in the Unites States. According to Center for Child Protection and Family Support, “Each year throughout the United States, over 3 million children are reported to state child protective services agencies. Nearly 1 million cases are substantiated” (n.d., p. 1). According to Jarvis (2015), there are four types of child abuse and neglect. They are: “neglect,” “physical abuse,” “sexual abuse,” and “emotional abuse” (p. 103). The age group I chose is preschool. Preschool children are common victims of emotional and physical abuse (Center for Child Protection and Family Support, n.d.). The most typical way that would lead a nurse to suspect child abuse is physical signs of Abuse. Nurses may not be able to precisely establish the age of the abuse but certain sizes and location can suggest the intention of the injury. For example, a nurse can detect that a child has been sexually abused because of “genital tissue injury” and/or “sexually transmitted infections” (Ricci, 2014, p. 406). Some other signs that would lead a provider to suspect child abuse are: “poor nutrition,” “severe malnutrition,” not brining child in for medical assistance, and “emotional neglect: rejection, indifference, and deprivation of love” (Ricci, 2014, p. 406) and (Blais, 2014, p. 610).

There are times nurses and physicians misidentify cultural variations of health practices as child abuse. For instance, “Coining or caogio, is an example of an ancient healing practice still being practiced, today. This dermabrasion therapy, which involves intense rubbing of the skin”…is use “to treat variety of illness” (Killion, 2017, p. 1). The intense rubbing leaves bruises on people. When nurses and physicians see bruises on children, like preschoolers, they assume child abuse. But Coining is a health practice for some South and East Asians. Moxibustion is another health practice that may be mistaken for child abuse. “…this health practice involves burning rolled pieces of moxa herb…directly over the skin above acupuncture points and allowing the herbs to burn near the skin’s surface until the onset of pain” (Killion, 2017, p. 2). Marks from this health practice on a child would also let the nurses think the child is being abused.

In New York, nurses are mandated reporters. As soon as a nurse suspect child abuse she or he must make a report by calling the New York Statewide Central Register of Child Abuse and Maltreatment (SCR) by phone. Mandated reporter call (800) 635-1522 to make a report (Office of Children and Family Services). According to New York State’s Office of Children and Family Services, “Oral reports to the SCR from a mandated reporter must be followed within 48 hours by a written report…” (2018, p. 3). An investigation will be done thoroughly after the report.
