There is one for the possible study participant to sign for themselves (those 18 years and older) combined with the Legally Authorized Representative (LAR) consent (parent) and one to perform an ASSENT- a child or adolescent between the ages of 12-17 years. I have placed comments in the consents to help you along.
In a separate file, answer this:
1. Discuss 2 challenges you discovered while completing the consents with your "actor" patients. Respond to other classmates.
2. Discuss 2 important lessons you learned to discuss with your classmates- pose a question to them at the end to stimulate discussion. Remember, you will not be able to see other classmates post until you post.
